Accepted Insurance

Insurances:

• Aetna
• Auto – you will need authorization from your case worker. We will need your claim number, date of loss, adjuster name and contact number for adjuster.
• Blue Cross Blue Shield – we do not accept Theramatrix (typically Ford Employees).
• BCN - need referral generated by your Primary Care Physician (PCP).
• Cigna
• Confinity – formerly PPOM
• Medicare – there is a cap on Physical Therapy benefits that typically covers 18-19 visits per year. If you have already had Physical Therapy this year, please let us know. If you do not have secondary insurance there will be a 20% co-insurance.
• Medicare – Railroad
• Multiplan
• Tricare
• United Health Care
• Workmans Comp - you will need authorization from your case worker. We will need your claim number, date of loss, adjuster name and contact number for adjuster.

We do not participate with:

• Aetna HMO
• HAP HMO
• Medicaid

If your insurance carrier does not appear on this list we encourage you to call the customer “service” number on the back of your card and let them know you would like to be seen at our office or by Angus Williams and they will tell you to what extent you’ll be covered.
